How to request/challenge a ban

If you are requesting a ban please include the following in your post:
Name of offender
Steam ID
A demo showing what he/she did that was unacceptable
If you are challenging a ban please include the following:
Your in-game name
Steam ID
The reason you got banned
Who banned you? (if you don't know, name the admins that were on the server)
Why you should be unbanned
